READ CAREFULLY - THIS AFFECTS YOUR LEGAL RIGHTS
In consideration for allowing the minor child identified above to participate in gymnastics classes, training, camps, recreational activities, sports, instruction, events, and other activities organized or provided by Du Sol Gymnastics School, I agree to the terms of this Agreement on my own behalf and in my capacity as the parent or legal guardian of the minor child identified above.
I understand that participation in gymnastics and related physical activities involves inherent risks that cannot be completely eliminated even when reasonable precautions are taken.
These risks may include, without limitation, falls, slips, collisions, loss of balance, improper landings, sprains, strains, fractures, dislocations, bruises, head or spinal injuries, paralysis, and other physical injuries, including serious injury or death.
I agree that I and my minor child will observe and obey all posted rules, safety instructions, warnings, and reasonable directions provided by Du Sol Gymnastics School and its employees, instructors, coaches, representatives, and agents.
I acknowledge and understand that gymnastics and related physical activities involve inherent risks. On behalf of myself and, to the extent permitted by applicable law, on behalf of my minor child, I voluntarily assume the risks inherent in participation in the activities provided by Du Sol Gymnastics School.
To the fullest extent permitted by applicable law, I release and discharge Du Sol Gymnastics School and its owners, officers, directors, employees, instructors, coaches, representatives, agents, and affiliates from claims arising from personal injury, death, or property damage resulting from risks that are inherent in the activities covered by this Agreement.
I understand that participation in gymnastics and physical activities may involve the possibility of injury. In the event of an injury or medical emergency involving my minor child, I authorize Du Sol Gymnastics School and its personnel, to the extent reasonably necessary, to obtain emergency medical assistance for my child when I or another authorized emergency contact cannot reasonably be reached.
I represent that, to the best of my knowledge, my child is physically capable of participating in the activities for which I am registering the child.
I agree to be responsible for property damage caused by me or my minor child through negligent, reckless, or willful conduct, to the extent permitted by applicable law.
I understand that photographs and/or video recordings may be taken during classes, events, camps, or other activities at Du Sol Gymnastics School.
This Agreement shall be governed by the laws of the State of Florida, to the extent applicable.
I acknowledge that I have had a reasonable opportunity to read and review this Agreement and that I am signing it voluntarily.
I certify that I am at least 18 years old and that I am the parent or legal guardian of the minor child identified in this Agreement, or otherwise have legal authority to provide consent and execute this Agreement on the child's behalf.
I HAVE READ THIS AGREEMENT COMPLETELY AND CAREFULLY, I UNDERSTAND ITS TERMS, AND I SIGN IT VOLUNTARILY AND WITH FULL KNOWLEDGE OF ITS SIGNIFICANCE.
